Upcoming Vaccine Clinics: COVID-19 Vaccine Screening and Consent Forms

In preparation of COVID vaccine clinics in our area that will first go to seniors, the local Family Health Teams are asking that seniors have their COVID-19 Vaccine Screening and Consent forms filled out and ready for the vaccine clinics.
The clinics are calling patients and instead of them all travelling to their respective FHT or physician office to pick up a form they will be able to pick them up in their communities. Forms are available in the foyer of the Municipal Office in Mindemoya from 8:30 am to 4:30 pm Monday to Friday.
